• Valid Passport ( must have atleast 6 months remaining validity ) and Old Passport.

• 4 pcs colored passport photo against white background (neutral face expression, mouth closed and no jewelries)

• For Employees: Certificate of Employment (must indicate position, date hired and salary).

> For Switzerland Visa Application, submit Leave of Absence.

(Start and end date based on the itinerary).

• Bank Certificate with Bank Statement or Passbook.

> Schengen Countries - current bank statement of the latest 3 months

> Italy - 1 year Bank statements.

• Travel Insurance (with COVID-19 coverage).

• Note: Additional documents are upon discretion of the Embassy.

Bring original documents on interview / appointment date.

Photocopies

• Business Documents for Self Employed. (Business Permit ,DTI or SEC Registration with General Info Sheet).

• Birth Certificate.

• Marriage Certificate.

• Income Tax Return.

> Employed – ITR 2316.

> Self Employed– ITR 1701 ( all pages).

> If no ITR – submit an explanation letter addressed to the Embassy.
